Planned and Preventative Healthcare
At Preston Family Medical, we believe good healthcare is not just about treating problems when they happen. Regular, planned care can help keep you well and prevent health problems from getting worse.
Chronic Condition Management Plans
If you have a health condition that has lasted, or is expected to last, 6 months or more, you may be eligible for a Medicare-funded Chronic Condition Management Plan.
Regular reviews can help your doctor:
These plans can usually be reviewed every 3 months.
Health Assessments
Medicare also supports health assessments for people at different stages of life or with particular health risks. These may include:
These appointments give your doctor time to look at your overall health, identify risks early and make a plan to help keep you well.
Talk to your doctor about whether you are eligible for a Chronic Condition Management Plan or health assessment.
Vaccinations
At Preston Family Medical, we believe prevention is an important part of good healthcare, and vaccinations help protect you at every stage of life.
We stock all government-funded vaccinations, for babies through to grandparents.
Your doctor can check which vaccinations are recommended for you based on your age, health conditions, lifestyle and vaccination history. We can also discuss additional vaccines that may be recommended but are not currently government funded.
Our doctors and wonderful nurses can administer your vaccinations conveniently at the clinic.
Vaccinations given at Preston Family Medical are recorded on the Australian Immunisation Register (AIR), so they can be viewed by other healthcare providers and accessed through your Medicare record in myGov.
Not sure if your vaccinations are up to date? Ask your doctor or our reception team.
